A Targeted Case Manager (TCM) provides assistance for children with a mental health diagnosis. The TCM can help them gain access to educational, medical, social, and other needed community services. The TCM helps to act as a liaison to link children and their families to services. The TCM meets with the child and family on a regular basis to assess needs and strengths to appropriately link them with services.
